New analysis highlights sensory dysregulation as key driver of behavioural issues in fragile X syndrome

A comprehensive study of over 1,400 children with fragile X syndrome reveals that sensory difficulties underpin multiple behavioural concerns, offering new targets for intervention and earlier diagnosis.

A new analysis of the largest clinic-based study of fragile X syndrome has given clinicians a clearer picture of how behavioural problems cluster in affected children, with attention difficulties and anxiety emerging as the most common concerns. The work, published in the Journal of Autism and Developmental Disorders, examined data from the FORWARD registry, a long-running United States database built to track the condition across specialist clinics.

According to the study team, the sample included 1,072 boys and 338 girls, an unusually large group for a rare genetic disorder. The researchers looked at eight behavioural concerns often seen in fragile X syndrome: attention problems, anxiety, hyperactivity, autism spectrum disorder, mood disturbance, irritability and aggression, self-injurious behaviour, and sleep problems. Autism was assessed using formal DSM-5 criteria, while the other concerns were clinician-identified behavioural comorbidities rather than formal diagnoses.

Attention problems and anxiety were the most frequently reported issues, but they were also among the least impairing. By contrast, disruptive behaviours such as irritability, aggression and self-injury were less common but had a much greater effect on daily functioning. Roughly half of children with any of the studied comorbidities had measurable impairment, underlining how often these difficulties become clinically significant.

The strongest finding was the extent of overlap: 69% of children had impairing behavioural problems that occurred alongside others, and 39% of those affected had more than two conditions at the same time. The pattern suggests that total behavioural burden matters more than any single diagnosis. The researchers also found that attention and hyperactivity tended to cluster together, as did anxiety and mood disturbance, echoing patterns seen in the general population.

Perhaps most notable was the finding that all eight behavioural problems were strongly linked with sensory difficulties. Earlier research from the CDC and other fragile X groups has highlighted anxiety, ADHD, autism and sensory dysregulation as common co-occurring concerns, while FORWARD studies have shown that hyperarousal to sound, touch and other stimuli can worsen behaviour and daily functioning. The new analysis suggests sensory dysregulation may be a shared driver across multiple symptoms, which could make it a useful target for future treatment.

The authors caution that the data came from specialty clinics and may not reflect the full range of fragile X syndrome in the broader population. Even so, they say the profiles could help clinicians spot emerging problems earlier and better tailor care for children and families. Because the FORWARD database is longitudinal and de-identified data can be shared under CDC policy, the researchers say it may also support future work on how these behavioural patterns change over time.
