Emerging research highlights that plain dairy products and breastfeeding have protective effects on dental health, but added sugars and prolonged feeding may increase caries risk. Unsweetened plant-based drinks pose varied effects, underscoring the importance of oral hygiene.
Milk, breastfeeding and milk alternatives are all common topics in dental counselling, yet the evidence is more nuanced than many patients assume. Research in children and adolescents suggests that plain milk and dairy products are generally not linked to higher levels of tooth decay, and may even help protect enamel. A study using US National Health and Nutrition Examination Survey data found that regular intake of milk, yoghurt, cheese and other dairy products did not raise caries risk, while a scoping review in the journal Children concluded that dairy can support remineralisation, inhibit cariogenic bacteria and help maintain a healthier oral environment.
That protective effect appears to depend heavily on whether the product is unsweetened. As the American Dental Association notes, the calcium in dairy can offset some of the cariogenic effect of lactose, but added sugar changes the picture altogether. Sweetened flavoured milks, fruit yoghurts and similar drinks can lose any benefit and become far more problematic for teeth. In practical terms, plain milk and plain dairy foods are one thing; sugary dairy beverages are another.
Breastfeeding is more complex still. Recent reviews indicate that breastfeeding in infancy is generally beneficial, and may even be associated with a lower risk of early childhood caries in the first year of life. But longer duration changes the balance. A systematic review and meta-analysis in Acta Paediatrica reported that breastfeeding beyond 12 months was associated with increased caries risk, while another review in Nutrients found that longer breastfeeding durations were linked with a higher likelihood of early childhood caries. The main message for clinicians is not to discourage breastfeeding in infancy, but to recognise that prolonged or frequent night feeding after the eruption of primary teeth may require closer oral-hygiene advice.
Milk alternatives are less straightforward than their healthy-image marketing suggests. A laboratory study from the University of Bern found that unsweetened oat, almond and soy drinks did not behave uniformly, with some showing effects on biofilms and oral bacteria that differed from cow’s milk. The results do not support a simple ranking of plant-based drinks as either better or worse for teeth. What does emerge clearly is that unsweetened versions tend to have lower caries potential than sweetened ones, but individual product composition still matters. For patients, the safest advice remains simple: choose unsweetened products where possible, and let oral hygiene do the heavy lifting.
