Brazilian study suggests personalised cannabis treatment may ease autism symptoms

A small Brazilian study suggests that individualised cannabis treatment may ease symptoms linked to autism spectrum disorder and improve family life, although the evidence remains preliminary and far from conclusive.

Researchers followed 20 patients with autism who received tailored daily doses of full-spectrum cannabis extracts, adjusting the balance of cannabidiol, or CBD, and tetrahydrocannabinol, or THC, according to each patient’s response. According to the paper published in Frontiers in Psychiatry, caregivers reported broad improvements after at least three months of treatment, including fewer emotional outbursts, less aggression, better communication and reduced distress in busy or noisy settings.

The findings add to a growing but still limited body of research on cannabis and autism. A systematic review of randomised trials found that while one study pointed to possible gains in overall symptom assessment, other outcomes were uncertain and the quality of evidence was low. Another review of the literature noted that only a handful of small studies and a few larger trials have examined the issue, making it difficult to draw firm conclusions about benefits or risks.

Still, earlier observational work has reported encouraging results. One study of 188 people with autism treated with medical cannabis found that most participants experienced at least moderate improvement after six months, while a separate review of nine studies suggested cannabis products may reduce hyperactivity, irritability, sleep problems, anxiety and aggression in some patients. Researchers say the biggest challenge is the wide variation in products, doses and study designs, which makes comparisons difficult.

The new study also reflected the practical appeal of a response-based approach. Most participants in the Brazilian research did not need THC, though some benefited from a low-ratio blend once dosing had been carefully adjusted. The authors said the results warrant further study, but they also acknowledged the limits of the work, including the small sample size and the absence of a control group.
