Early signs of menstrual pain risk identified in young girls, revealing social disparities and potential for earlier intervention

New research indicates that girls experiencing painful periods in adolescence often show warning signs years earlier, with disparities linked to ethnicity and social factors. Early identification could lead to better support and reduce the impact of menstrual pain on young girls’ lives.

Girls who develop painful periods in early adolescence may already be showing warning signs years beforehand, according to research from the long-running US Adolescent Brain Cognitive Development study published in The Lancet Child & Adolescent Health.

The study followed 2,254 girls aged 9 and 10 who had not yet started menstruating and checked back when they were 12 or 13. By then, nearly 58% reported dysmenorrhoea, the medical term for period pain, and about one in five of those said the pain was severe. Researchers found that girls who later went on to have painful periods were more likely, at age 9 or 10, to have had nausea, dizziness or other physical symptoms, as well as signs of earlier pubertal development such as breast growth, body hair or a growth spurt.

Sleep problems at that age did not appear to predict whether a girl would later develop period pain. But among those who did, poorer sleep earlier in childhood was linked to greater disruption once menstruation began, suggesting sleep may influence how seriously symptoms affect daily life. A separate body of research has similarly linked poor sleep, later chronotype and shorter sleep duration with worse menstrual problems in early adolescence.

The findings also point to unequal burdens. Black adolescents were 38% more likely than White adolescents to develop painful periods, while Hispanic adolescents were 34% more likely than non-Hispanic adolescents. Other recent research has found that structural stigma is associated with earlier pubertal timing among Black and Latinx youth, supporting the idea that social stressors may help shape when puberty begins. The authors of the menstrual-pain study said these disparities may reflect broader differences in access to care, stigma around menstruation and earlier physical development.

The researchers argue that the pattern they observed could help doctors identify girls at higher risk before their first period starts, creating a chance for earlier support. That matters because adolescent period pain can interfere with school, sleep and everyday activity, and studies of early teenage pain have linked more widespread pain complaints with a heavier symptom burden and lower quality of life. For parents and caregivers, the message is that period pain should not be dismissed as an inevitable part of growing up, particularly when symptoms are severe or begin to affect normal routines.
