Study of 10 million siblings reveals subtle links between birth order and disease risks

A pioneering analysis of over 10 million American siblings uncovers modest yet significant associations between birth order and the likelihood of developing certain health conditions, highlighting the complex interplay of familial and environmental factors.

A large study of more than 10 million American siblings suggests that birth order may be linked to differences in the risk of certain diseases later in life, although the researchers stress that it does not show one position in the family causes another. The analysis, based on two decades of health insurance records and published in Nature Health, examined 569 disease categories and found statistically significant differences in 150 of them. The pattern points to birth order as a modest population-level marker, not a diagnosis or prediction for any individual child.

According to the research, first-born children were more likely to be diagnosed with a range of neurodevelopmental and psychiatric conditions, including autism, attention-deficit/hyperactivity disorder, Tourette’s syndrome and obsessive-compulsive disorder. They also showed higher rates of immune-related conditions, including food allergies and hay fever. Second-born children, by contrast, were more likely to have substance use disorders, gastrointestinal conditions, migraines and shingles. Similar findings were described in Scientific American and other reports on the preprint, which also noted that the differences were statistically significant but relatively small.

The study’s authors took care to compare siblings within the same families as well as children from different families, in an effort to reduce the influence of household income and other background factors. Even so, the results cannot untangle every possible explanation. Health claims data can reflect differences in when families seek care or how often doctors are consulted, and first-time parents may be more likely to pursue evaluation when they notice something unusual. Researchers and clinicians quoted in coverage of the study also pointed to possible roles for parental age, spacing between pregnancies, immune changes and early environmental exposure.

The findings add to a broader scientific debate about how much family position matters for long-term health. Earlier research, including a study of multiple sclerosis in Canada, found no meaningful link between birth order and that disease, showing that not every condition follows the same pattern. For parents, the practical message is limited: birth order may be associated with small shifts in population risk, but it is not a reliable way to forecast a child’s future health, and factors such as genetics, environment and access to care remain far more important.
