New research links birth order to distinct patterns of childhood health conditions

A comprehensive U.S. study finds associations between a child’s birth position and their later risk of neurological, behavioural, and allergic conditions, suggesting biological and social influences but emphasising the need for further research.

A large U.S. study suggests birth order may be linked to a child’s later health, with first-born and second-born siblings showing different patterns of diagnosis across a wide range of conditions. Researchers analysing insurance claims for more than 10 million people from over 5 million two-child families found statistical associations across 150 of 418 disease categories with enough cases to assess, according to the report discussed by Knowridge and the underlying research published in Nature Health. First-born children were more likely to be diagnosed with autism, attention-deficit hyperactivity disorder, Tourette syndrome, tic disorders, asthma, hay fever, food allergies, depression and anxiety. Second-born children were more likely to be diagnosed with substance-use disorders, migraine, shingles, gastritis and some joint problems.

The findings fit with earlier work suggesting that younger siblings may be less prone to some allergic conditions, possibly because they are exposed earlier to germs brought home by older brothers and sisters. Researchers have long argued that this kind of early microbial exposure can shape the immune system, which may help explain why birth order has sometimes been tied to asthma and allergies. But the new analysis is broader than those earlier observations, extending the question beyond infection-related illness to developmental, neurological and behavioural conditions as well.

Related studies point to several possible biological and social pathways. A meta-analysis from the Pregnancy and Childhood Epigenetics Consortium found birth-order-related differences in neonatal DNA methylation, a form of epigenetic marking that can affect how genes are switched on and off. Separate research in the British Journal of Cancer reported that higher birth order was linked with a modest rise in breast cancer risk and a lower melanoma risk, while another study in Scientific Reports examined whether birth order influenced the risk of traumatic brain injury in children. Taken together, the research does not prove a single mechanism, but it suggests birth order may leave measurable traces in health across the life course.

The authors of the new study stressed that the results do not show birth order causes disease. They said the differences for any one child were small and that genes, lifestyle, access to care and wider environment are likely to matter far more. They also noted that first-time parents may watch older children more closely and seek assessments earlier, which could increase diagnosis rates for conditions such as autism or ADHD. Because the study relied on claims data, differences in healthcare use and detection may also have shaped the findings, so further research will be needed before birth order can be treated as an independent health risk factor.
