A comprehensive analysis of nearly 10 million US siblings uncovers subtle associations between birth order and a spectrum of health diagnoses, prompting questions about environment, biology, and family dynamics, though causation remains unproven.
A large study of nearly 10 million US siblings has found that birth order is linked to a wide range of health conditions, though researchers say the pattern does not mean family position causes disease.
Scientists from the University of Chicago and Columbia University examined 569 disease categories across sibling groups and identified 150 conditions with statistically significant associations. In findings published in Nature Health, first-born children were more likely to receive diagnoses including autism, ADHD, Tourette’s syndrome and obsessive-compulsive disorder, as well as some allergic and immune-related conditions such as food allergies. Later-born children showed higher rates of migraines, shingles, substance use disorders and some gastrointestinal problems, including gastritis.
The study adds to a wider body of research suggesting that birth order can be associated with differences in biology and health, but not in a way that can predict an individual child’s future. Earlier research published in Nature Communications by the Pregnancy and Childhood Epigenetics Consortium found birth order-related differences in newborn DNA methylation, hinting at possible immune and blood-pressure pathways. Other studies have also reported links between birth order and outcomes ranging from childhood traumatic brain injury to some cancers and childhood leukaemia survival, though those studies likewise stop short of proving causation.
Experts caution that claims data can blur the picture because diagnosis rates depend on how often families seek care and how closely children are assessed. Differences in parental age, pregnancy conditions and spacing between births can also shape outcomes. Fox News medical analyst Marc Siegel said the study raises useful questions rather than delivering firm answers, describing the findings as a starting point for investigating how environment, family dynamics and biology may interact.
