Environment-focused risk score outperforms genetics in predicting adolescent substance use

A recent Nature study introduces a new environmental risk score, T2SU-PXS, that more accurately predicts adolescent substance-use vulnerability than family history or genetic scores, highlighting the influence of social and household factors in early drug exposure.

A Nature study has found that a data-driven environmental risk score can predict adolescent substance-use vulnerability more effectively than family history alone or a genetic score for addiction risk, underscoring the role of social and household conditions in shaping early exposure. Using data from 9,771 young people in the Adolescent Brain Cognitive Development cohort, researchers built a Time-to-Substance-Use PolyeXposure Score, or T2SU-PXS, to capture cumulative environmental liability during a period when experimentation and escalation can begin to take hold.

The model identified six main environmental factors linked to earlier substance use, spanning young people’s behaviour, the home environment and peer and community influences. The authors said the score was designed through an exposome-wide approach, which scans a wide range of exposures rather than focusing on a single risk factor. In their analysis, the T2SU-PXS performed better than family history density of substance use and a polygenic score for addiction risk, suggesting that the environment may be more measurable and actionable than previously captured in developmental psychiatry.

That conclusion fits a long line of research showing that the age at first use matters. Studies published in Nature and other journals have found that people who begin using drugs in adolescence face a greater likelihood of progressing to dependence than those who start later in life. Earlier work in JAMA Psychiatry also showed that both inherited and shared environmental influences affect tobacco initiation, alcohol use and use of other drugs, with peer pressure, access to substances and sibling behaviour all playing a role.

Other studies have pointed to the importance of neighbourhood and family context. Research indexed in PubMed found that adolescents in higher-risk communities were more likely to use substances, partly because of how they perceived risk and their willingness to take it. A separate review in PMC highlighted child maltreatment, parental influence and peer effects as major drivers of accelerated alcohol and drug involvement. The CDC says cannabis use disorder affects about 3 in 10 people who use cannabis, with risk higher among those who begin in youth or adolescence, reinforcing why prevention efforts are often aimed at delaying first use.
