New D-DAND tool offers deeper insights into the complex neuropsychiatric profile of Dravet syndrome

A new caregiver interview scale, the D-DAND, promises enhanced monitoring of neuropsychiatric and daily functioning issues in Dravet syndrome, moving beyond seizure-focused evaluations.

Dravet syndrome is often described through its seizures, but the disorder’s burden reaches far beyond epilepsy. A systematic review published on PubMed found that executive dysfunction, expressive language impairment and psychiatric difficulties are common in Dravet syndrome, reinforcing the need for longitudinal, multidimensional assessment rather than a seizure-only view of the condition. Researchers in the epilepsy paper published in Epilepsia have now proposed a caregiver interview designed specifically to capture those wider day-to-day problems.

The new instrument, called the Dravet Disease-Associated Neuropsychiatric Disorders scale, or D-DAND, was built to measure the areas that matter most in routine care: adaptive skills, communication, motor function, behaviour, emotion, sleep and feeding. In the study, clinicians recruited 123 patients with confirmed Dravet syndrome at centres in Italy and France, then asked caregivers to complete a 63-item interview that could be finished in around 35 minutes. The aim was to create a practical tool that can track change over time and avoid the ceiling and floor effects that limit many general-purpose developmental scales.

The scale was developed in stages, beginning with an international expert panel and repeated rounds of refinement with clinicians, neuropsychologists and a family representative. The final version groups items into subdomains and broader domains, allowing clinicians to build a structured profile of a patient’s abilities and difficulties. That approach reflects a wider move in Dravet care towards tools that are specific to the syndrome. A related project known as DANDA is also working on a tailored checklist, while the DANCE screening tool, reported separately, focuses on identifying neuropsychiatric comorbidities rather than grading severity.

The Epilepsia study found that D-DAND produced high short-term test-retest reliability, with the overall score showing particularly strong stability over roughly two weeks. The researchers also tested how D-DAND compared with established scales such as the Vineland Adaptive Behavior Scales and the Child Behavior Checklist. Correlations ranged widely, which the authors say is evidence that some familiar instruments overlap with Dravet-specific problems, while others miss important features such as feeding, sleep and subtler behavioural change. That pattern fits the broader literature, which suggests that Dravet-related impairments are complex, uneven and evolve over time.

The authors argue that the scale could help clinicians follow progression, identify patients who need more detailed neuropsychological or sleep assessment, and document whether change is linked to disease course, treatment or intervention. It is not meant to replace seizure tracking or compare children with typically developing peers. Instead, it is intended as a syndrome-specific monitoring tool for families and care teams who need a clearer picture of how Dravet syndrome affects everyday life.
