A case of central precocious puberty in a young girl highlights the growing trend of earlier puberty onset, driven by genetic, environmental, and lifestyle factors, amid advances in diagnosis and treatment.
Gabby was six when she had her first period and began developing body hair, prompting her mother to seek medical help. According to Gabby, speaking to the BBC World Service, paediatric staff had to look beyond their own hospital because they had never encountered a case like hers. She was eventually diagnosed with central precocious puberty, a condition in which puberty begins before age eight in girls and before age nine in boys.
Specialists say the condition can be caused by the brain starting the hormonal process too early, although in many children no clear trigger is found. The U.S. National Institute of Child Health and Human Development says central precocious puberty may be linked to brain abnormalities, tumours or genetic factors, while peripheral precocious puberty has different causes, including hormone-producing tumours or exposure to external hormones. Mayo Clinic gives a similar breakdown and notes that the signs can include early breast development in girls and enlarged testicles in boys.
Gabby said the months of tests and the need for regular injections made her feel different from other children. Doctors placed her on gonadotrophin-releasing hormone blockers, a treatment that pauses pubertal development until a more typical age. The NIH Genetic Testing Registry says normal puberty usually starts much later than it did in Gabby’s case, and Endotext explains that central precocious puberty results from early activation of the hypothalamic-pituitary-gonadal axis, the hormonal control system that drives sexual development.
Researchers say girls are affected far more often than boys, and recent studies suggest the age at which puberty begins may be falling in some populations. A Danish study of more than 8,500 children found higher incidence rates in girls and boys over time, while a 2025 meta-analysis reported that the average age of breast development in girls declined by about three months per decade between 1977 and 2013. In Brazil, endocrinologist Ana Pinheiro Machado Canton told the BBC that obesity may be one factor speeding up puberty, alongside biological and environmental influences.
Genetics can also play a role. Endotext and other research published by the National Center for Biotechnology Information describe MKRN3 mutations as an important cause of familial and some sporadic cases of central precocious puberty. In the BBC report, Canton said genetic testing helped her team identify a family mutation that meant one younger sister was highly likely to develop the condition as well. For families, the key message from doctors is that early recognition matters: treatment can limit the emotional strain of growing up too soon and may reduce longer-term risks linked with untreated early puberty, including shorter adult height and higher chances of later cardiovascular and hormone-related cancers.
