President Donald Trump’s proposal to separate the MMR vaccine has reignited debate over the practicality and safety of administering combined vaccines, with experts warning of potential setbacks to immunisation rates in the United States.

The answer, according to the CDC and vaccine historians, is mostly practical. Combination vaccines reduce the number of injections children need, make schedules easier for families to follow and help raise vaccination rates.
The CDC says the MMR vaccine has been used in the United States for decades and is designed to make immunisation more convenient without sacrificing safety or effectiveness. The agency recommends two doses, with the first given between 12 and 15 months of age and the second between 4 and 6 years. In practice, that means two appointments and two injections. Splitting the protection into three separate vaccines would mean more visits, more needles and, public health experts say, a greater chance that children would miss one of the doses.
Experts quoted by WRAL said there is no published evidence showing any advantage to breaking MMR into three separate shots. Dr Walter Orenstein, who once led the U.S. immunisation programme, said the original vaccine was possible because all three diseases use weakened live viruses and share similar manufacturing requirements. Dr Amesh Adalja of Johns Hopkins said separating the shots would add inconvenience and raise the risk that one vaccine would “slip through the cracks”. The CDC also notes that combination vaccines are tested before approval to make sure they are as safe and effective as individual vaccines.
The idea is not supported by current vaccine practice in other countries either. According to the United Kingdom government, no country recommends routine use of three separate vaccines for measles, mumps and rubella. Some countries allow separate vaccines in limited situations, such as after exposure or for travel, but still use the combined shot in standard childhood schedules. France and Japan have limited exceptions, though both continue to rely largely on combined or partial combination approaches rather than a universal split.
Manufacturing a new set of separate vaccines for the United States would not be quick. Merck told PolitiFact that splitting MMR would amount to creating three new experimental products, each requiring full clinical testing and regulatory review, and estimated that it could take more than 10 years before approval and commercial production. Public health experts warn that if the combined shot were withdrawn before replacements were ready, vaccination would effectively stop. Even if separate vaccines were eventually approved, they say uptake would likely fall, with more cases of measles, mumps and rubella as a result.
