A large-scale sibling study analysing 10.3 million individuals reveals that birth order correlates with varying risks for neurodevelopmental, allergic, and other health conditions, prompting fresh insights into family health dynamics.
A vast sibling study has found that birth order is associated with different patterns of disease risk, with first-born children more likely to be diagnosed with some neurodevelopmental and allergic conditions and second-born children more likely to face other health problems, including substance misuse disorders, migraines and some gastrointestinal illnesses. The analysis drew on health records from 10.3 million people in 5.1 million families and examined 569 conditions, making it one of the broadest looks yet at how family rank may relate to later health.
Researchers reported statistically significant birth-order links for 150 diseases among conditions with enough cases for analysis. First-born children had higher odds of autism, ADHD and food allergy than second-borns, while the opposite pattern appeared for several other disorders, including substance abuse, migraine and biliary tract disease. Scientific American reported that the paper also found excesses among first-borns for allergic rhinitis, anxiety or phobic disorders, tics and Tourette syndrome.
The findings, however, do not show that being first born causes any of these conditions. The study is observational and is currently available as a preprint, meaning it has not yet been peer reviewed. The authors said the associations could reflect a mix of biological differences between pregnancies, parental monitoring, diagnostic timing and social or environmental influences within households.
One of the study’s strengths was its use of sibling comparisons within the same families, which helps limit some of the confounding that can affect research comparing unrelated children. The authors also ran a separate matched between-family analysis of about 1.6 million sibling pairs, and the two approaches generally pointed in the same direction for many of the key results. That consistency does not prove causation, but it does suggest the pattern is unlikely to be a statistical fluke.
The paper adds to earlier research suggesting that birth order can shape health in more modest ways over the life course. A Norwegian study cited by the National Bureau of Economic Research found first-born adults were more likely to have high blood pressure and be overweight, while later-born siblings reported worse self-rated health and different health behaviours. Separate Austrian research published through IZA found later-born children were generally healthier at birth, but first-borns used more medical care and received more preventive attention.
Even so, the new analysis should not be read as a prediction tool for individual families. Birth order is only one factor among many that can affect health, and the study used U.S. insurance-claims data, which may not reflect diagnosis patterns elsewhere. For parents, the practical takeaway is simple: symptoms matter more than sibling position, and concerns about development, allergies or other illnesses should be discussed with a doctor rather than inferred from where a child falls in the family order.
