Emotional reactivity identified as key factor in early autism symptom structure

A study of nearly 1,900 preschool children with autism reveals emotional reactivity as a central component, paving the way for more personalised support strategies and highlighting the disorder’s diverse subgroups.

A study of nearly 1,900 preschool children with autism spectrum disorder has found that emotional reactivity sits at the centre of a wider web of emotional and behavioural symptoms, underscoring the case for earlier and more tailored support.

Using Child Behaviour Checklist data from 1,886 children with autism and 285 age-matched typically developing children, researchers mapped how symptoms connected with one another rather than looking at each in isolation. The network analysis showed that children with autism had a clearly different symptom structure from their typically developing peers, with emotional reactivity emerging as the most influential feature in the autism group.

The study also identified two distinct subgroups within the autism sample. Both differed sharply from typically developing children across emotional and behavioural measures, but they were not identical to each other. The researchers found that the two autism subgroups had different overall levels of symptom connectivity and distinct links between specific behaviours, especially between aggression and withdrawal, suggesting that preschool autism is not clinically uniform.

That fits with earlier research showing substantial overlap between core autism features and emotional or behavioural difficulties. A previous study of 335 preschool children found that emotional, behavioural and repetitive problems formed a separate component from social communication deficits, and that both were linked with parental stress. Other work has also pointed to strong connections between sensory processing problems and behavioural difficulties in young autistic children, while separate validation studies have shown that the Child Behaviour Checklist can help identify autism in several settings, including German-speaking, Korean and low- and middle-income populations.

Taken together, the findings add weight to the argument that emotional dysregulation is not just an added complication in autism but part of the disorder’s early clinical picture. The authors say the results may help guide more individualised intervention, with one subgroup pointing towards broad early intervention and the other suggesting a greater need for functional communication training and strategies that support social engagement.
