A Wuhan-based study suggests that maternal overweight and glucose response patterns during pregnancy may influence early mental development in girls, highlighting potential sex-specific vulnerabilities linked to maternal metabolic health.
A prospective Chinese cohort has found that girls born to mothers who were overweight or obese before pregnancy and who also showed the strongest glucose response on an oral glucose tolerance test were more likely to have poorer mental development at age 2. The study, based on data from 2,639 mother-child pairs in Wuhan, adds to growing evidence that maternal metabolic health during pregnancy can shape early neurodevelopment, though the authors say the broader associations were modest and in some cases did not survive stricter statistical correction.
The research drew on women who received routine antenatal care between May 2014 and May 2019 and had a standard 75-g glucose tolerance test at 24 to 28 weeks of gestation. Their children were later assessed with the Bayley Scales of Infant Development at age 2, with mental and psychomotor scores converted into standard developmental indices. The investigators grouped maternal glucose responses into three patterns rather than relying only on the usual gestational diabetes diagnosis, arguing that this approach captures more detail about how glucose changes after the test load.
In the wider sample, maternal pre-pregnancy overweight or obesity was linked to slightly lower mental development scores, and girls in particular showed this pattern, but those findings did not remain significant after correction for multiple comparisons. The glucose-response patterns on their own were not clearly tied to poorer developmental outcomes. By contrast, the combination of maternal excess weight and the most pronounced glucose pattern was associated with a lower mental development score in girls and a higher likelihood of suboptimal mental development. The same joint effect was not convincing in boys.
The results fit with a broader body of research suggesting that maternal obesity can influence child cognition, behaviour and executive function. A recent umbrella review covering 78 studies and more than 650,000 mother-child pairs found links between maternal obesity and lower cognitive performance, weaker executive function and more ADHD symptoms in offspring. Earlier work has also reported poorer problem-solving, fine motor skills and executive functioning in children exposed to very severe maternal obesity during pregnancy.
The Wuhan study cannot prove cause and effect, and the authors caution that the findings may be influenced by residual confounding, the single-region hospital-based design and the limited glucose measurements available from routine care. Even so, the paper suggests that looking beyond a simple gestational diabetes diagnosis may help identify pregnancies in which maternal metabolic stress is more likely to affect early child development. The strongest signal was in girls, but the researchers say larger studies with longer follow-up will be needed to confirm whether the effect is genuinely sex-specific or reflects chance variation.
