Early-life sugar restriction linked to reduced dementia risk in later life

A UK study suggests that limiting sugar exposure during early childhood may have long-term protective effects against dementia and Alzheimer’s, challenging previous assumptions about sugar consumption and brain health.

A study of people born in Britain during and just after the last years of post-war sugar rationing suggests that stricter exposure to sugar in early life may be linked to better brain health decades later, including lower rates of dementia and Alzheimer’s disease. The work used the end of the wartime rationing system in September 1953 as a natural experiment and analysed health records from the UK Biobank, one of the country’s largest long-running population studies.

Researchers examined 64,737 participants born between 1951 and 1956, dividing them into those exposed to sugar rationing in utero and early childhood and those born after restrictions had ended. According to the report in Neurology, the strongest associations appeared in people who experienced rationing in the womb and through the first year or two of life. In those groups, the risk of all-cause dementia was lower than in the non-rationed cohort, and the risk of Alzheimer’s disease was also reduced.

The benefits were not limited to diagnosis rates. The study found later onset of dementia, Alzheimer’s disease and vascular dementia among those with early-life sugar restriction, along with signs of better brain structure and performance on several cognitive tests. MRI data from a subcohort showed higher total grey matter volume and fewer white matter hyperintensities, which are linked to small-vessel brain damage. The researchers also reported better results in tasks measuring processing speed and reasoning.

The findings add to a growing body of UK Biobank research suggesting that sugar exposure may influence dementia risk across the life course. Separate studies from the same resource have reported that higher intakes of total sugars, free sugars and sugary drinks are associated with a greater risk of dementia, while another analysis found a dose-response relationship between sugar intake and dementia risk that was partly shaped by genetic susceptibility. A broader natural-experiment study published in a separate journal also linked early-life sugar rationing not only to lower dementia risk but to reduced depression and anxiety later in life.

The new paper remains observational, so it cannot prove that sugar restriction itself prevents dementia. Still, lead investigator Jiazhen Zheng of The Hong Kong University of Science and Technology said the results suggest that nutrition in the earliest stages of life may have lasting effects on the brain. The study also hinted that later type 2 diabetes and hypertension may explain part of the relationship, pointing to possible long-term pathways between early diet and neurodegenerative disease.
