Birth order linked to lifelong health risks, study reveals

A large-scale study suggests that a child’s position in the family may influence their risk of developing various health issues later in life, highlighting the potential impact of family dynamics on long-term wellbeing.

A child’s place in the family may be linked to the likelihood of developing a range of health conditions later in life, according to a large study published in Nature Medicine on August 3. Researchers from the University of Chicago and Columbia University analysed health records covering more than 10 million children with at least one sibling and found that birth order was associated with differences in several diagnoses, though not in a way that proves cause and effect.

The strongest pattern was seen in neuropsychiatric conditions. First-born children were more likely to be diagnosed with autism, attention-deficit hyperactivity disorder, Tourette syndrome and obsessive-compulsive disorder, while younger siblings more often had digestive problems, migraine and some other conditions. Out of 569 health outcomes examined, the researchers found statistically meaningful differences in 150, suggesting the effect was broader than a handful of isolated disorders.

The team tried to limit the influence of family income and home environment by comparing siblings within the same family as well as children from different families. Even so, the authors stressed that the findings show association rather than proof that birth order itself drives disease. They suggested several possible explanations, including differences in parental age, attention and expectations, and greater exposure to microbes among younger children as they grow up alongside older siblings.

The study also found that the spacing between births may matter. A larger age gap between siblings was linked with a higher autism risk among first-born children, but with fewer allergies in that group. Separate research cited in Nature has also highlighted the role of genetic and familial factors in autism risk, reinforcing the view that apparent environmental links often need to be interpreted cautiously. Researchers said more work is needed to untangle the biological, social and environmental mechanisms behind the pattern.
