Study reveals the passive exposure of teenagers to self-harm content online via algorithmic feeds

A new Oxford-led study shows that nearly a third of adolescents in England encounter self-harm material passively on social media, raising concerns over the influence of recommendation algorithms on youth wellbeing.

Social media has become the main arena in which teenagers chat, learn and pass the time, but a new Oxford-led study suggests that the same systems which serve up music, jokes and new friendships can also deliver content about self-harm with alarming ease. Published in Nature Mental Health, the research found that 34.5% of adolescents aged 11 to 18 in England had seen self-harm material online in the previous month, and most had not gone looking for it.

The study drew on responses from more than 32,000 pupils taking part in the OxWell Student Survey, a large-scale project that tracks the wellbeing of school-age children and young people. Researchers said the key concern was not merely how many teenagers encountered such material, but how it reached them. In most cases, exposure was passive: the content appeared in feeds rather than being actively searched for.

According to the study, 72.9% of those exposed said they encountered the material while scrolling, and around two-thirds said it came via algorithmic recommendations. Only a small minority reported deliberately searching for self-harm content. Dr Holly Bear, the study’s lead author, told Medical Xpress that the findings show this is not an obscure corner of the internet, but part of many teenagers’ everyday online environment.

The research also found that adolescents exposed to self-harm content reported higher levels of anxiety, depression and loneliness than those who had not seen it. However, the authors stressed that the data do not prove cause and effect. It remains possible that young people already struggling emotionally are more likely to notice, seek out or engage with this kind of material. Even so, the findings raise difficult questions about recommendation systems designed to maximise attention without always accounting for psychological risk.

The Oxford study sits alongside a separate British analysis, published in JAMA Network Open, that complicates the wider debate over screen time. In that work, researchers from the University of Edinburgh and Imperial College London examined 3,786 children in the Growing Up in Scotland cohort and found no meaningful link between time spent on screens at ages five or ten and later psychosocial difficulties. At 15, a connection appeared only for very heavy video-game use. The implication, researchers said, is that public-health advice focused purely on how long children spend online may be too crude, because what they see and how platforms shape that experience may matter more than minutes alone.
