A recent assessment by epidemiologist Gideon Meyerowitz-Katz questions the reliability of studies linking artificial sweeteners to cognitive decline, emphasising the need for cautious interpretation amid broader evidence that sugar remains the more significant health risk.
Popular alarm over sweeteners and brain health has outpaced the evidence, according to an assessment of recent research by epidemiologist Gideon Meyerowitz-Katz. The controversy was fuelled by a Brazilian cohort study that appeared to link higher sweetener intake with faster cognitive decline, but the claim has been widely questioned because of the way the data were gathered and interpreted. As Meyerowitz-Katz argues, the headline-grabbing conclusion is far less secure than it first sounded.
The biggest weakness lies in the quality of the exposure data. The Brazilian study relied on food-frequency questionnaires, which ask people to remember what they ate over long periods, rather than measuring actual intake. It also assumed that participants’ diets stayed broadly unchanged over eight years, an assumption that is difficult to defend in any real-world population. On top of that, the analysis lumped together chemically different substances , from sugar alcohols such as xylitol to synthetic sweeteners such as aspartame , as though they had the same biological effect. That raises the risk of drawing a false conclusion from a mix of unlike compounds.
There is also a more basic problem of causation. People with diabetes, cardiovascular disease or other metabolic problems are more likely to switch to low-calorie sweeteners in the first place. That means sweeteners may be a marker of existing illness rather than the cause of later cognitive decline. Reuters-style caution is warranted here: association is not the same as proof, especially when independent reviewers have pointed to statistical and mathematical errors, and when the full-group analysis did not show the dramatic link that the headline version suggested.
What the broader evidence does show is that sugar remains the clearer health hazard. The Cleveland Clinic notes that both sugar and artificial sweeteners can reinforce cravings, but it also stresses that excess sugar is tied to obesity, metabolic syndrome, type 2 diabetes, hypertension and heart disease. The National Cancer Institute says the main sweeteners approved by the US Food and Drug Administration have undergone extensive safety review and have not been shown to cause cancer in people. The World Health Organization, meanwhile, has concluded that non-sugar sweeteners may help reduce body weight in the short term when used in place of sugar, but that long-term benefits are uncertain.
That does not mean sweeteners are a magic health food. The WHO says there is no evidence of long-term harm at intakes within acceptable daily limits, but it also does not recommend relying on them as a permanent solution for weight control. Research cited by the National Institutes of Health suggests the gut can distinguish between sugar and artificial sweeteners, helping to explain why zero-calorie substitutes may not fully satisfy cravings. Even so, the practical conclusion is straightforward: for most people, replacing sugary drinks with a sweetened zero-calorie version is likely to be less damaging than sticking with sugar, particularly if the swap helps with weight and blood sugar control.
