A preliminary study suggests that altering gut bacteria through microbiota capsules could help some people with peanut allergies tolerate more of the allergen, marking a potential step toward new treatments, but research remains in its early stages.
Researchers are exploring whether changing the gut microbiome could make peanut allergy less severe, after an early clinical trial suggested that capsules containing donated microbes may alter immune responses in some patients. In a study funded by the National Institutes of Health and led by Boston Children’s Hospital, 15 people with peanut allergy swallowed oral capsules carrying gut microbes from donors without food allergies. Six of them later tolerated more peanut during supervised testing without reacting.
The findings are preliminary, but they add weight to a growing idea in allergy science: that the community of bacteria and other microbes in the digestive tract may help shape whether the immune system learns to tolerate food or treats it as a threat. Scientists have long found differences between the gut microbiomes of people with food allergies and those without them, although that does not prove cause and effect. A 2022 systematic review of 12 studies concluded that evidence remains limited and that larger, better-controlled research is needed before firm conclusions can be drawn.
In the NIH-supported trial, 10 participants received the microbiota capsules alone, while five first took antibiotics in an attempt to make it easier for the incoming microbes to take hold. Four months later, three people in each group could eat a larger amount of peanut during medically supervised food challenges without an allergic reaction. Researchers also reported immune changes in those who responded, including a rise in a type of regulatory T cell linked to immune tolerance, which stayed elevated for as long as a year.
That does not mean the treatment is ready for routine use. The study was too small to show whether the approach works reliably, who is most likely to benefit, or how long any effect lasts. A separate paper published in August 2026 reported similar findings in a study of 15 people with peanut allergy and also suggested that gut microbes from responders helped protect mice from food allergy, pointing to possible roles for bile acid metabolites in oral tolerance. But that, too, remains early-stage research.
For now, the work is best seen as a clue rather than a cure. Fecal microbiota transplantation, or FMT, is already used in medicine for some cases of recurrent Clostridioides difficile infection, but it carries risks and requires strict donor screening. Food challenges should only be done in a medical setting, and people with peanut allergy should not try to recreate the study on their own with probiotics, transplants or deliberate peanut exposure. The longer-term goal, researchers say, may be a more targeted therapy that captures the helpful microbial signal without transferring an entire donor microbiome.
