A new CDC survey reveals stagnant HPV vaccination rates among American teens for a record fourth year, with rural areas facing the starkest disparities in vaccine coverage and uptake.
A new federal survey suggests HPV vaccination among American teenagers has stalled for a fourth straight year, while coverage for two other routine adolescent shots edged down. According to reporting on the CDC’s National Immunisation Survey-Teen, the latest findings come from provider-verified records for 18,692 adolescents aged 13 to 17 and were published in the Morbidity and Mortality Weekly Report on 3 September.
The national figures show 77.7% of teens had received at least one dose of HPV vaccine and 63.4% were up to date on the series. Coverage for the Tdap shot, which protects against tetanus, diphtheria and pertussis, slipped to 88.8% from 91.3% the previous year. MenACWY, the vaccine that guards against several forms of meningococcal disease, also declined slightly, to 89.0% from 90.1%.
The broad national picture hides sharper problems in rural communities. Teens in mostly rural areas were 12 percentage points less likely than those in mostly urban areas to have received at least one HPV dose, and 14.2 points less likely to be fully up to date. The rural-urban gap for HPV remained even after the CDC team split the data by household income, suggesting poverty alone does not explain the difference. By contrast, the smaller gap for MenACWY appeared only among adolescents living below the poverty line.
That pattern points to a mix of barriers, rather than a single cause. The CDC authors noted that teens in rural areas are less likely to attend the well-child visits where vaccination often begins and may be less likely to receive a clear recommendation from a clinician. Lower acceptance of vaccines in rural communities has also been documented elsewhere, which helps explain why HPV coverage continues to lag even where cost is less likely to be the main obstacle.
The survey also shows stark differences between states and some large cities. Rhode Island reported the highest HPV coverage, while Mississippi had the lowest. Several major metros, including Chicago, Philadelphia, New York City and Houston, posted stronger HPV numbers than the rest of their states, although the report did not test those differences for statistical significance. The CDC said the Tdap and MenACWY declines should be read cautiously because the survey response rate was low and provider records were incomplete for many participants.
