Weekend physical activity dips reveal need for tailored support for childhood cancer survivors

Research uncovers distinct weekend drops in activity among childhood cancer survivors, highlighting a targeted window for intervention and personalised follow-up, especially for adolescent girls and CNS tumour survivors.

More than a third of childhood cancer survivors in two age groups tracked by researchers saw their physical activity fall away at weekends, a pattern the authors say offers a clear target for support rather than another vague call to “exercise more”. In the peer-reviewed study, adolescent girls who had survived central nervous system tumours stood out as the subgroup most likely to sit in the least active pattern across the week, prompting the researchers to argue for more tailored follow-up in survivorship care. The paper, published in the Journal of Cancer Survivorship, drew together three datasets: adolescent survivors and controls from the Physical Activity in Childhood Cancer Survivors study, adult survivors from the Swiss SURfit trial, and adult controls from the Norwegian National Physical Activity Survey 2. The version of record was published on 24 July 2026. Using hip-worn ActiGraph monitors and a latent class model that analysed moderate-to-vigorous and light physical activity together, the team sorted 434 adolescent survivors aged 9 to 18 and 156 adult survivors aged 18 to 49 into weekly movement patterns rather than relying on weekly averages alone. Among adolescents, 20% fell into a low, steady pattern of roughly 30 minutes of moderate-to-vigorous activity and 140 minutes of light activity a day, well short of the usual 60-minute daily benchmark for young people. Another 32% managed about 60 minutes of more intense activity on weekdays before dropping to around 30 minutes at weekends. A smaller 8% group built up activity through the school week and peaked on Friday before falling back, while the biggest group, 41%, stayed close to 60 minutes of moderate-to-vigorous activity and 200 minutes of light activity every day. The adult picture was different. The largest class, 40%, followed a medium regular pattern with about 40 minutes of moderate-to-vigorous activity and 250 minutes of light activity a day. A further 31% showed the same weekend drop seen in adolescents. Another 23% kept moderate-to-vigorous activity steady but logged around 350 minutes of light activity daily; the authors said that split may reflect the difference between manual work and office-based jobs. Only nine adults, or 6%, landed in the most active group, posting high weekday totals before a weekend dip. The paper also found that the adults in the high-weekday, low-weekend class were disproportionately male, more likely to sleep only seven hours on weekdays, and more likely to have had a haematopoietic stem cell transplant. When the researchers compared survivors with controls, the surprise was not that survivors were uniformly less active. In adolescents, survivors were less likely than controls to be in the low regular class, 20% against 31%, and more likely to sit in the medium bands. In adults, no equivalent low-moderate-to-vigorous class appeared among survivors, while 17% of controls fell into one. The adult survivor and control groups were similarly likely to occupy the medium regular and medium weekend-drop patterns, but survivors were more likely to be in the class with regular moderate activity and unusually high light activity. Adherence figures also cut against the stereotype of a largely inactive survivor population: 35% of adolescent survivors met activity recommendations, compared with 29% of controls, while the corresponding figures in adults were 84% and 72%. The authors added that survivors’ weekly curves appeared steadier than controls’, a difference they said might reflect more planned behaviour or “energy economization”. (link.springer.com)

Who ended up in the least favourable groups mattered as much as the percentages. Adolescent survivors in the low regular class were more likely to be female, older at assessment, sleeping seven hours on weekdays, and survivors of central nervous system tumours. They were also less likely to come from families with higher parental education than adolescents in the more stable medium regular class. Among adults, higher education was concentrated in the medium regular group, whereas lower education was more common in the class with especially high light activity. Taken together, the paper suggests sleep and social background may be nearly as important as diagnosis in shaping how survivors move through the week. (link.springer.com)

The findings come with important caveats. The authors said accelerometers are poor at capturing activities with little vertical movement, including cycling, strength training and water-based exercise, and noted that one or two weeks of monitoring may not fully represent usual behaviour. Sedentary time was not included in the modelling. They also warned against over-reading the adult survivor versus control comparison because SURfit participants had volunteered for an exercise trial, which may mean they were unusually motivated. Low inclusion rates, 19% in SURfit and 26% in the Norwegian adult survey, and the lack of adjustment for wear time or season mean the reported differences are descriptive rather than definitive. The study also lands as part of a broader research programme rather than a one-off observation. Oslo University Hospital’s profile for first author Mari Bratteteig lists earlier PACCS papers on device-measured activity and cardiovascular risk in 2022, physical activity in adolescents in 2023, and activity behaviour and screen time in younger survivors in 2024, showing a steady shift from asking how much survivors move to asking when and how they do it. An earlier abstract in the programme book for the 4th International Symposium on Late Complications after Childhood Cancer, held in Seattle from 19 to 21 June 2025, had already hinted at the same question with more provisional class labels and percentages, suggesting the published analysis was refined before journal publication.
